In order for us to open the Sheriff's file we need to have the following documents:
• An ORIGINAL Letter of Instructions to the Sheriff
• An ORIGINAL and 5 copies of the WRIT OF EXECUTION
• An Original and 5 copies of the APPLICATION FOR EARNINGS WITHHOLDING ORDER (WAGE GARNISHMENT);
• 3 Copies of the following: EMPLOYER’S RETURN; EARNINGS WITHHOLDING ORDER (please see attached Sample for additional requirements); EMPLOYEE INSTRUCTIONS
A check payable to the Sheriff in the amount of $25.00
After service has been effected we will file the appropriate PROOFS OF SERVICE with the Sheriff.
